Acute and Chronic Health Management

AVAD created a referral network system designed to guide our individuals who require alternative services not provided directly by us.

We are pleased to facilitate connections with esteemed medical practitioners, and community partners, who focuses on prevention, diagnosis, treatment and management of various health conditions.

​Acute illnesses are conditions that come on suddenly and typically last for a short duration. They often require immediate medical attention and range from mild to severe. Examples of acute illnesses include the flu, a broken bone, or a heart attack. Treatment is usually focused on alleviating symptoms and may involve medications, surgery, or other interventions.

 

In contrast, chronic illnesses are long-lasting conditions that can be managed but not usually cured. These illnesses develop gradually and persist over time, often requiring ongoing medical care and lifestyle adjustments. Common examples of chronic illnesses include diabetes, arthritis, and heart disease. Managing chronic illnesses often involves an approach that includes medication, regular check-ups, and lifestyle changes aimed at improving quality of life.
